Position Summary

The Nurse Coordinator is responsible for coordinating all clinical functions, nursing practice and the standards of care related to hospice services for patients/families at City of Hope. Works closely with physicians, nurses, clinical social workers, case managers, and supportive care team to help create a smooth transition of care for patients and families. Educates patients, families, City of Hope staff, and the community about resources and requirements available. Plans, implements, coordinates, and evaluates patient care in assigned department or program. Performs all duties within the scope of licensure utilizing the nursing process. Acts as a patient advocate in all ethical and legal aspects.This opportunity is designated for full-time(5 shifts per week, Monday-Friday), 8-hour shifts, and day shifts.

As a successful candidate, you will:

  • Utilizes the City of Hope nursing process in clinical practice
  • Establishes plan of care for home infusion therapy under the direction of the attending physician.
  • Plans and coordinates all diagnostic testing, patient assessment, treatments and follow up home care.
  • Interacts with patients and caregivers, obtains information and assesses the need for psychosocial support. Assists patient and caregiver in developing coping mechanisms.
  • Functions as clinical resource and liaison to all members of health care team.
  • Develops and or presents educational programs for patients, significant other, and staff related to specific program or departmental needs.
  • Attends and participates in meetings as required.
  • Performs other related duties as assigned or requested.

Your qualifications should include:

  • Bachelor’s Degree or Master’s degree in Nursing is required
  • Three (3) years recent oncology experience
  • One (1) year previous home health experience
  • Current California RN license
  • Basic Life Support (BLS) CPR Card through American Heart Association
  • Valid CA Driver's license and proof of insurance

Salary / Pay Rate Information:
Pay Rate: $64.07 - $92.91 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
